Facility Evaluation Report
Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met Assistant Administrator David Berry at approximately 12:00 PM and explained reason for visit.
This facility is licensed as an Adult Day Care facility to serve seventy (70) Developmentally Disabled clients ages 18 and above, eight (8) may be non-ambulatory. The property consists of one large building with reception area, administration offices, three classrooms, two kitchens, client storage room, facility storage room, staff room with lockers.
Facility walls and floors were in good condition, adequate lighting, fire extinguishers were properly charged. All toilets and hand washing facilities were well maintained, safe, sanitary and in operating condition. Men’s restroom water temperature measured at 127,6 not within the required reading of 105-125 degrees. Both kitchen areas were clean and well maintained. LPA observed a one gallon bottle of multi surface liquid sanitizer unlocked under kitchen sink deficiency cited. Refrigerators are available for client use to store their lunches. The program does not provide lunch but does provide snacks.
Five (5) staff files were reviewed and included Criminal clearance record, CPR/training, and health screening with TB. Five (5) client files were reviewed and included physicians report, TB clearance, and individual program plan (IPP)report. Last fire/earthquake drill was conducted in September of 2024. Infectious control plan was reviewed. Three (3) staff and three (3) clients were interviewed. Two (2) client medications were reviewed. Medications are centrally stored and locked MAR log is used.
Deficiencies have been noted on LIC 809D under Title 22 Regulations. Exit interview was conducted and a copy of this report, LIC 809D and appeal rights were provided to Administrator.
