The inspector’s account

Investigation revealed the following: Regarding allegation Staff sexually abused client, it was alleged that C1 was sexually abused while in care.

Interviews with Administrator and staff revealed that C1 does not attend to this Program. Administrator stated that C1 has been a client of C-H Adult Day Center Program which is across the street. LPA review of both C-H Adult Day Center Program and C-H Adult Day Center Program #3 client rosters revealed that C1 attended C-H Adult Day Center Program and LPA review of C1's file also revealed that C1 was admitted to C-H Adult Day Center Program on 09/09/21 and was the client of this program until 11/07/25. Interviewed Administrator and staff denied the allegation that C1 was sexually abused by the staff. Administrator stated facility staff at both programs never abuse clients, physically, emotionally or sexually. Interviewed staff stated that staff have never sexually abused C1 or other clients. They have never witnessed any staff sexually abused C1 or other clients. Interviews with clients indicate they have never been sexually abused by staff nor have they witnessed staff sexually abusing other clients and were unable to corroborate the allegation. Interviewed Administrator mentioned that on 07/30/25 they had an Informal Conference with RC related to the anonymous sexual allegation complaint involving client ( C1) and staff. Regional Center conducted the investigation, and alleged sexual abuse / assault was found inconclusive / unsubstantiated and investigation was closed on 08/07/25. Copies of investigation documents were provided to LPA. Review of documents and facility files shows that staff whose names were mentioned in Informal Conference Forms, employees of C-H Adult Day Center Program not C-H Adult Day Center Program #3. However, it has not been proven that they sexually abuse the client. Interviewed RC representative stated that investigation is closed and during investigation process determined that alleged sexual assault is unsubstantiated.

Based on observation, review and interviews, investigation revealed: Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

Exit interview was conducted and a copy of this report was provided.
