The inspector’s account

Licensing Program Analysts (LPA) Sandra Urena arrived at the facility unannounced to conduct a required annual visit. The LPA called the Licensee Vardan Adzemian on the phone and informed them of the visit. The licensee arrived 15 minutes later. The LPA met with the Licensee and explained the reason for the visit.

Currently the facility has zero (0) clients and is awaiting to be vendored through North Los Angeles County Regional Center (NLACRC). Licensee will inform the LPA of final approval as a vendor for the NLACRC and LPA will conduct a case management visit to ensure physical plant areas inside and outside to ensure there are no health and safety hazards.

The LPA, along with the Licensee toured the physical plant areas inside and outside to ensure there are no health and safety hazards. COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition. The facility maintained a comfortable temperature. Smoke detector(s) and carbon monoxide detector were tested and operational at the time of the visit. The fire extinguisher was fully charged and was last purchased on 08/10/2025. Required postings were observed throughout the common space. KITCHEN: Knives are stored in a locked kitchen drawer. Kitchen appliances are in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Medications will be located in a locked top cabinet adjacent to the the kitchen. First aid kit is also located in this cabinet. BEDROOMS: Residents' bedrooms were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level. RESTROOMS: Restrooms are relatively clean and sanitary and in operating condition. The laundry units are located in a room adjacent to the the kitchen area. OUTDOOR SPACE: The back patio which has a covered outdoor area for client use. There is a gate on the side of the house designated for an emergency exit. The LPA observed cameras at the exterior perimeter of the facility. There are no bodies of water on the premises. The property is connected to a city approved an Accessory Dwelling Unit (ADU). The clients will not have access to the ADU.

No deficiencies were observed at this time. Exit interview conducted and a copy of the report was issued.
