Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit on 11/07/2025 and was greeted by two staff members.. LPA stated the purpose of the visit was to conduct an annual inspection. The staff members confirmed there are three residents. The facility is licensed for four residents.

LPA reviewed the client records at 9:00 am until 9:45 am. LPA reviewed the staff files at 9:45 am until 10:15 am.

LPA and the staff toured the facility at 10:15 until 10:45 am. During LPA's visit, LPA observed the facility was neat and clean.

Common Areas – LPA observed the living room contained comfortable seating. The dining room is furnished with table and chairs. The family room contained comfortable seating and a television.

Hallway Closet - LPA observed the closet was locked and contained resident medications, first aid kit, cleaning solutions, and knives.

Kitchen – LPA observed a two-day supply of perishable foods and a seven-day supply of nonperishable foods. LPA observed there were no safety issues in the kitchen.

Water Temperature - LPA tested the water temperature at 10:30 am which was 105.1 degrees F.
