Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit and was greeted by two staff members. LPA stated the purpose of the visit was to conduct a case management visit. The Administrator, Christopher Singleton arrived at 8:00 am. The Administrator confirmed there are three clients.
An incident report was received by Community Care Licensing stating a client (C1) had an unexplained injury. LPA interviewed two staff members (S1-S2) from 8:15 am until 8:30 am. LPA reviewed client files from 8:30 until 8:45 am.
S1-S2 confirmed C1 will injure themselves when they are upset. S1-S2 also stated they will document the incidents and redirect C1 when necessary. At 8:30 am, LPA observed the three clients were clean, dressed and eating breakfast during the visit. LPA observed there were no injuries on the clients.
LPA toured the facility from 9:45 am until 10:00 am.
Based upon LPA' s observations, staff interviews, and review of clients' files, there are no deficiencies to report at this time.
Exit interview conducted and a copy of the report was given.
