Facility Evaluation Report

Licensing Program Analyst (LPA) Trevor Byrne and North Los Angeles Regional Center Community Service Specialist Liseth Carrillo (CSS) conducted a collateral visit at the facility today. LPA arrived to the facility at 12:00 PM and CSS later arrived at approximately 01:15 PM. LPA met with facility staff #1 (S1) who contacted the facility Administrator Nicole Whaley via telephone call. The Administrator was unable to come to the facility during today's visit but has designated S1 to sign this report on their behalf. Entrance interview conducted with the Administrator over the phone and the reason for the visit was explained.

During today’s visit, the LPA and CSS conducted an interview with one (1) client between approximately 12:05 PM and 02:00 PM.

This report was read to S1 as the Administrator was unavailable. Exit interview conducted. Report was reviewed and a copy was provided.
