Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced case management visit and was greeted by the Resident Care Director and the Business Office Manager. LPA stated the purpose of the visit was to review residents' files. LPA confirmed LPA’s receipt of reports relating to an incident that occurred at the facility.

LPA Spaeth toured the facility at 10:00 am until 10:45 am.

LPA interviewed the Resident Care Director and Business Office Manager at 11:00 am until 11:15 am. LPA reviewed residents' files 11:20 am until 12:00 pm. LPA Spaeth requested and received copies of the staff work schedule, resident roster and residents' documents.

There are no deficiencies to report at this time. Exit interview conducted and a copy of the report was given.
