Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced case management visit and was greeted by two staff members. LPA Spaeth stated the purpose of the visit was to tour the facility, and review resident and staff records. There are two residents living at the facility.

LPA reviewed the resident's files at 10:30 am until 11:00 am LPA Spaeth reviewed the staff files at 11:50 am until 12:00 pm.

LPA Spaeth toured the facility at 12:00 pm until 12:15 pm. LPA also observed the staff work schedule and the administrator certificate were posted within the facility.

There are no deficiencies to report at this time. Exit interview was conducted and a copy of the report was given.
