Facility Evaluation Report
On 9/08/2025 at approximately 9:11 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an unannounced annual visit to the facility. LPA was greeted by the Administrator Tracey Tindle and stated the reason for their visit.
LPA asked for the census. LPA conducted a physical plant tour at approximately 09:30 AM and the following was noted:
The facility is a single unit building with four (4) bedrooms and two (2) bathrooms currently occupying zero (0) clients. There is no designated staff room. The facility has an approved fire clearance for four (4) ambulatory clients. The facility is awaiting vendorization through North Los Angeles Regional Center (NLARC) to operate at a Level VI Adult Residential Home.
Common areas: The living room, dining room and Family room were observed to be properly furnished and in good repair. The facility maintains a comfortable temperature at 84°F. LPA observed a fire extinguisher to be located near the kitchen and dated 08/22/2025. LPA observed required postings such as See/Say Something and the Facility Sketch to be located throughout the common areas. A working telephone was observed. LPA observed the fireplace to be covered and inaccessible to clients.
Kitchen: The kitchen was observed to be free from pests. The Administrator will ensure sufficient supplies of seven (7) day nonperishable foods and two (2) day perishable foods will be readily available upon the arrival of clients. The knives/sharps were observed to be kept within a locked box inside of a locked filing cabinet located near the entrance. Laundry Room: The laundry appliances were observed to be located near the Family Room. The laundry appliances were observed to be working and in proper condition. The cleaning solutions/toxins were observed to be stored within the garage. (continued on LIC 809-C)
