Licensing Program Analyst (LPA), Melissa Spaeth conducted an unannounced visit to the facility. Upon arrival, LPA knocked on the door but there was no activity at the facility. LPA spoke to the Licensee, Hilewna Tamrat who stated there are no residents at the facility. The Licensee arrived at 9:30 am.
LPA Spaeth and the Licensee toured the facility 9:30 am until 9:40 am. I observed there are no residents living in the facility.
Exit interview conducted and a copy of the report was given.
