Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced case management visit. LPA was greeted by two caregivers. LPA Spaeth stated the purpose of the visit was to conduct a case management visit since the facility will be sold to another Licensee.
LPA Spaeth observed a third person at the facility and asked staff about the third person. The staff stated a family member was visiting them and confirmed the family member has not completed the finger print clearance process. LPA Spaeth called the Administrator, David Salvador and stated the visitor would need to leave the facility. LPA also explained to the staff members the visitor would need to leave as soon as possible. LPA observed the visitor left at 12:00 pm.
LPA Spaeth reviewed the residents' files at 11:00 am until 11:30 am. LPA toured the facility at 12:00 until 12:15 pm. LPA observed gardening tools in the backyard and I instructed the caregiver to lock up the items. LPA reviewed staff files at 12:15 pm until 12:35 pm. Pursuant to Title 22 Division 6 of the CA Code of Regulations, a deficiency was cited (refer to LIC 809-D).
Exit interview was conducted, Appeal Rights discussed, and a copy of the report was given.
