Facility Evaluation Report

Licensing Program Analyst (LPA) Evelin Rios conducted a case management visit to confirm the closure of this facility. LPA contacted the licensee and later met with Emmanuel A Manabat II at the facility. On 07/06/26, LPM Naira Margaryan received an email from the licensee regarding the facility closure.

During today's visit, LPA conducted a physical plant tour and confirmed that there were no residents at the facility. LPA obtained the License. The licensee initiated the closure of the facility.

LPA will submit the file for closure upon returning to the Woodland Hills South Regional Office. The effective closure date is 07/21/2026.

An exit interview was conducted, and a copy of this report was signed and provided to the licensee.
