Facility Evaluation Report

At 11:00 a.m., Licensing Program Analyst (LPAs) Shira Stamps and Melissa Ruiz conducted a case management visit with Licensee Felix Campos in conjunction with a complaint visit. Entrance interview conducted.

The purpose of the case management is to address deficiencies observed during the complaint visit. LPAs conducted interviews and conducted a physical plant tour. LPAs completed a document review for Administrator Certificates for the current staff members. It was found that the facility does not have a certified Administrator. Therefore, based on document review the facility currently has no certified Administrator.

Exit interview conducted. Citations issued, appeal rights and copy of report delivered to Licensee.
