The inspector’s account

Licensing Program Analyst (LPA) Jose Tan conducted an unannounced case management visit at this facility to check on health and welfare of the residents. Upon arrival at around 9:48 AM, LPA was greeted and met by the Licensee Felix Campos and explained the reason for the visit.

LPA conducted physical plant tour at around 10:03 AM and observed that the four (4) clients are in the facility and per the Licensee, one resident was brought to the hospital yesterday due to eye problem.

Additionally, LPA explained that the other purpose of the visit was to tour the facility and confirm if the deficiencies that were issued on the last visit of 6/11/2025 have been corrected.

During previous visit of LPA Spaeth and LPM Agard on 6/11/2025, the licensee was informed that the two (2) staff members working at the facility had not been fingerprint cleared and the individuals needed to immediately leave the facility. The Licensee stated the staff member had been asked to move out of the facility, but the staff member refused to leave and was still living on the premises. Per the Licensee, they are still living at the premises.

During the tour, LPA observed there was a sufficient supply of food. However, the following deficiencies have not been corrected by the Licensee:

80075 Health Related Services: On 6/11/2025, a client was observed using oxygen with no professionally trained staff present. The Licensee did not provide proof to LPA during this visit.
