Facility Evaluation Report
On 6/19/2025, Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced case management visit. Upon entering, LPA observed three clients were in the facility and there were no staff members present. The Licensee Felix Campos arrived at 9:40 am and LPA asked why the clients were left alone. The Licensee explained they had only been away from the facility for a few minutes.
LPA explained the purpose of the visit was to tour the facility and confirm if the deficiencies that were issued on the last visit of 6/11/2025 have been corrected.
During LPA's previous visit on 6/11/2025, LPA Spaeth and LPM Agard had informed the Licensee that the two staff members working at the facility had not been fingerprint cleared and the individuals needed to immediately leave the facility. As of today, one of the individuals walked into the facility. The Licensee stated the staff member had been asked to move out of the facility, but the staff member refused to leave and was still living on the premises
LPA toured the facility at 9:45 am until 10:00 am. LPA observed there was a sufficient supply of food. However, the following deficiencies have not been corrected by the Licensee:
- 80075 Health Related Services: On 6/11/2025, a client was observed using oxygen with no professionally trained staff present. The Licensee did not provide proof to LPA Spaeth by 6/19/2025 that the staff were properly trained.
