On 6/06/2025, Licensing Program Analyst (LPA) Melissa Spaeth and Licensing Program Manager, Troy Agard conducted a complaint investigation for Complaint # 31-AS-20250605130645. During LPA's visit, LPA and LPM both observed two staff members who have not been fingerprint cleared.
LPA Spaeth and LPM Agard stated to the Administrator the staff members must immediately leave the facility.
Due to LPA’s and LPM’s observations, the following deficiency was issued (see 809-D).
Exit interview conducted, appeal rights discussed, and a copy of the report was given.
