Facility Evaluation Report
On 4/23/2025, Licensing Program Analyst (LPA) Melissa Spaeth and Evelin Rios initiated a complaint investigation for Complaint #31-AS-20250423102042. During the visit, LPA’s toured the facility at 1:15 pm until 1:30 pm and observed rotten food in a broken refrigerator. LPA also observed food was not covered in the refrigerator and there was not a two-day supply of perishable food items. Also, the floors and bathrooms were dirty, a resident's door was broken, and a window was broken. A client's medication was stored in the refrigerator but was not properly locked. The backyard contained debris of wood and other items, which could be a potential hazard for clients in care.
Based upon LPA's observations, the following deficiencies are issued.
Exit interview conducted, appeal rights given and a copy of the report was given.
