Licensing Program Analyst (LPA) Evelin Rios conducted an unannounced visit. LPA arrived at the facility and knocked on the door. Nobody answered. LPA contacted the licensee Felix Campos and requested him meet LPA at the facility. LPA Rios explained the reason for the visit. Felix met with LPA.
On 01/28/2019 LPA Michael Cava documented the same citation twice on LIC809-D. LPA Cava corrected the citation by removing the duplicate. On todays visit LPA Rios obtained a signature on a hard copy of the LIC809-D dated 1/28/2019. A copy of the LIC 809-D was left with Licensee.
Exit interview conducted, and a copy of this report was given.
