Facility Evaluation Report

LPA Spaeth conducted an unannounced visit and was greeted by caregiver LPA and Caregiver toured the facility at 8:00 until 8:15 am. LPA observed the facility has a seven day supply of non perishable food and a two day supply of perishable food.

The Administrator arrived at 8:10 am. LPA stated the purpose of the visit was to conduct an unannounced visit. The Administrator stated that the second bathroom has a leak and repair work is being completed on the tub area. LPA observed the first bathroom is properly working and is available for residents to use.

There are no deficiencies to report. Exit interview conducted, and a copy of the report was given to the Administrator.
