Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Spaeth conducted an unannounced visit and was greeted by the Administrator, Jamelene Enola. LPA Spaeth stated the purpose of the visit was to inspect the additional physical plant changes there were completed based upon the Regional Center's request.

LPA observed a wooden cover was installed over the bathtub in the bathroom located in Room 2 and the bathroom door was not locked. The Administrator stated the wooden cover was added for the safety of the residents.

Two gates were added to the surrounding wall separating the facility from the additional open property on the south side of the property and the west side of the property. LPA did not obseve animals on the property. LPA did not observe an RV parked in the open property area. Based upon LPA' observation of the surrounding property, clients are able to exit the property in case of an emergency.

Based upon LPA's observations, there are no issues to report at this time.
