Facility Evaluation Report

On 03/19/2026 at approximately 09:30 AM, Licensing Program Analyst (LPA), Angelica Segovia conducted an annual visit to the facility. LPA was greeted by the designated staff member, Jennifer Goodson and stated the reason for their visit.

LPA asked for the census. LPA conducted a physical plant tour at approximately 09:40 AM and the following was noted:

The facility is a two-story building with three (3) bedrooms and three (3) bathrooms currently occupying zero (0) clients. The facility has an approved fire clearance for four (4) ambulatory clients. The facility is currently non-operational and is under construction due to water damage. Additionally, once all repairs have been completed, the facility will begin their process of becoming vendorized through North Los Angeles Regional Center (NLARC) to operate as an Adult Residential Home.

There were no immediate health and safety hazards observed during the day of inspection. Exit interview was conducted and a copy of this report was provided to the designated staff member for today's visit.
