Facility Evaluation Report

Licensing Program Analyst conducted an unannounced visit to the location. LPA knocked on the door but no one answered the door. Per LPA's observation there were no residents in the facility at the time of this visit.

LPA Spaeth spoke to the Licensee, Tony Horne who stated there are no clients living in the facility. The Licensee stated the facility will be closed due to personal issues the Licensee was facing. LPA stated Community Care Licensing would need to complete a tour of the facility in order to confirm there are no clients occupying the property.

The License confirmed they were unable to meet LPA Spaeth at the location.
