Facility Evaluation Report
On 05/06/26, at 08:55am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, annual visit. LPA met with caregiver, Rita Banks and advised Austin Shanks , Administrator of the visit. Austin Shanks, Administrator arrived about ten (10) minutes later.
LPA asked for the census, client, and staff files.
The facility has been licensed as a Adult Residential Facility. There are currently five (5) clients that reside at the facility. The physical plant was toured inside and outside. It is a single, story home.
Kitchen area was sufficiently stocked with seven (7) days of perishable and seven (7) days of non-perishable food. There is one (1) refrigerator in the kitchen area in the main house and several other refrigerators that are also in the shed area in the backyard. There are several canned goods including canned goods in the shed area. There are two (2) fire extinguishers fully charged. One (1) is in the kitchen area and the other one (1) is in the hallway in between the rooms. Sharps/Knives and chemicals are kept secured and locked towards the back of the kitchen. There is one (1) washer and dryer towards the back of the kitchen.
The medication is kept by the kitchen area locked and secured, inaccessible to the clients.
