Facility Evaluation Report
Licensing Program Analyst (LPA) Abeye Duguma met with Verjine Egian for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 09:30 AM and the following was noted:
This is a commercial property with front and rear entry points. There is outdoor furniture with a shaded area for clients and parking for visitors. The facility does not have a swimming pool/body of water.
This is an Adult Day Program vendored through the North Los Angeles Region Center. The facility services developmentally disabled clients and their normal operating hours are Monday through Friday from 9:00a.m. to 3:00p.m. The facility currently has a staff to client ration of one to four (1:4).
The facility is fire cleared for ninety (90) developmentally disabled clients of which nine (9) may be non-ambulatory. The facility is currently providing services to fifty-four (54) clients.
This facility has a large well-lit open space with multiple activity zones ranging from arts and crafts to gym to media. The common areas are neat and clean. There are two (2) bathrooms each with multiple stalls and wash areas. The bathroom was checked for cleanliness and proper operations. Residents have enough personal hygiene products provided by the licensee.
