Facility Evaluation Report

On September 20, Licensing Program Analyst (LPA), Deborah Lee conducted an unannounced annual required visit using the CARE Inspection Tool. LPA Lee met with Ivy G. Redjal and the purpose of today’s visit was explained. This facility is licensed to serve (4) Non-Ambulatory Adults, ages 18 through 59. Facility currently has 4 clients serviced by Harbor Regional Center.

The facility is a single-story home located in a residential area in Norwalk, Ca. A tour of the facility includes 2 common rooms, dining area, kitchen, 4 client bedrooms, 1 client bathroom, 1 staff bathroom, staff resting room, detached garage, laundry area, front yard and back yard.

LPA inspected all 4 bedrooms; the mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked.
