Facility Evaluation Report
Licensing Program Analyst (LPA) Nune Margaryan made and unannounced Health and Safety check visit in response to Death of Licensee Alvin Fabian. Upon visiting LPA met with Lolita Espiritu. Sheila Fabian notified about the visit over the phone and arrived shortly after. LPA explained the purpose of today's visit. At the time of visit there was 1 client and 2 staff present at the facility. 2 clients were at Day Program.
On July 28, 2025, OD, LPA Mary Flores received a call from Sheila Fabian (Family member / sister of Licensee) who reported about passing of Licensee Alvin Fabian. Sheila Fabian indicated that she has a current Administrator Certificate and associated to the facility and willing to take over the facility.
Sheila Fabian is interested in continuing operating the above Adult Residential Facility and on 07/29/25, she submitted an application to Centralized Application Bureau (CAB) for review / processing. Copies of application and Certified mail receipt were provided to LPA.
During today’s visit LPA toured the facility. Food supply was checked. Medications were centrally stored and locked. No immediate health & safety concerns were observed.
Exit interview was conducted with Sheila Fabian and the copy of the report was provided.
