The inspector’s account

Licensing Program Analyst’s (LPA’s) Christian Gutierrez, and Elena Mallett conducted an unannounced Case Management Visit to follow up on a special incident report (SIR) we received on 09/29/2025. LPA's was met by Administrator John De La Cruz and explained the purpose of the visit.

Licensing received an SIR for Client #1 (C1) stating client had an unexplained bruise on left side of face and eye. C1 was at hospital on 09/08/2025 -09/12/2025, transferred to Sniff on 09/12/2025 - 09/19/2025, and released back to facility on 09/19/2025. Bruise was observed by staff on 09/20/2025.

During today's visit LPA spoke with Administrator, obtained staff rooster, client rooster C1’s face sheet, physicians report and two years of individual program plan IPP. No concerns, obstructions, or anything out of the ordinary was witnessed during the visit. LPA’s were unable to interview staff at time of visit. LPA’s obtained staff phone numbers to conduct interviews at a later time. LPA Gutierez attempted to interview C1 over the telephone at day program. LPA will return if a follow-up visit is needed.

No deficiencies observed during today's visit. Exit interview held and a copy of the report will be emailed due to LPA printer not working.
