Facility Evaluation Report

Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced Plan of Correction (POC) visit to follow up on deficiencies cited during annual visit conducted on 9/9/25. Jon De La Cruz, Administrator was contacted telephonically and the reason for the visit was explained. Florinda Aleman, Direct Support Professional, assisted with the visit due to the administrator having prior engagements.

The purpose of the visit is to follow up on POCs due on 9/10/25. The following deficiencies were corrected during time of visit:

CCR Section 80019(e)(2): proof of criminal background clearance for renter/tenant was provided by administrator. LPA obtained verification of identification and CCLD confirmed criminal background clearance for renter/tenant.

CCR Section 80044(a)(2): LPA was granted access to the room in the sectioned-off garage which is currently occupied by the renter/tenant.

CCR Section 80010(a): Civil Penalty was assessed today for Failure to Correct for $1,300. Licensee is operating a business other than an Adult Residential Facility (ARF) which poses an immediate health and safety risk for clients in care. Licensee/Administrator did not submit a plan showing how the staff will maintain capacity as an ARF and acknowledge the intent of the regulations to maintain compliance. Immediate attention is warranted.

Exit interview was conducted via telephone call with Jon De La Cruz, Administrator and with Florinda Alena, Direct Support Staff in person and copies of the reports, POC letters, civil penalty and Appeal Rights were provided at the time of visit.
