The inspector’s account

Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced annual continuation visit stemming from the annual visit conducted on 9/9/25 which was not completed due to facility having (2) active COVID cases. LPA met with Florinda Aleman, Direct Support Professional and the reason for the visit was explained. Jon De La Cruz, Administrator, arrived thereafter and assisted with the visit.

During today’s visit, LPA Cota, conducted a record review of (4) client and (3) staff files, tested the smoke and carbon monoxide detectors, reviewed facility’s infection control plan and Emergency Disaster Plan. Records reviewed had all relevant documents in place. Smoke and carbon monoxide detectors were tested and working properly. Infection Control and Emergency Disaster Plans are up to date.

No deficiencies cited today under California Code of Regulations, Title 22, and California Health and Safety Code. Exit interview was conducted with Jon De La Cruz, Administrator, and a copy of the report was provided.
