Licensing Program Analyst (LPA) Elena Mallett conducted a Case Management visit at the facility and was met by Case Manager Michelled Delgado. The purpose of the visit was explained. LPA had experienced computer issues on previous 04/24/26 Case Management visit and returned today to obtain signatures for 04/24/26 visit and provide licensing report.
On today's visit a staff and client roster was obtained. LPA toured the physical plant and no health or safety concerns were observed.
An exit interview was conducted with Case Manger Michelle Delgado and a copy of this Licensing report was provided
