Facility Evaluation Report
On 10/12/2025, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced 1 year required visit at the facility above. LPA Richard met with staff Rose Rasco. LPA explained the purpose of today visit.
The house consisted of 4 bedrooms, 1 staff room 1 staff bathroom, 1 client bathrooms, living room, kitchen, dining area, den and an attached garage where the washer and dryer is located. There is another house located in the backyard premises for the administrator to reside. LPA toured inside and outside the facility. LPA observed toured the kitchen, and observed 7 days of perishables and 2 days nonperishable.
The front and backyard are well maintained, and the passageways and exits are free of obstruction. The resident bathrooms are clean, and showers have non-skid materials. The hot water temperature measured at 115.5 degrees F, there is sufficient lighting throughout the facility.
