Facility Evaluation Report

Licensing Program Analyst (LPA) Nicol Wesley conducted an unannounced 1 year required visit at the facility and met with staff Lolita Manzanilla Espiritu who contacted Administrator Sheila Ann Fabian, and at that time the purpose of the visit was discussed. The facility phone number is 562 864 4799.

The house consisted of 4 bedrooms, 1 staff room 1 staff bathroom, 1 client bathrooms, living room, kitchen, dining area, den and an attached garage where the washer and dryer is located. There is another house located in the backyard premises for the administrator to reside. Passageways and exits are free of obstruction. LPA toured the kitchen and observed 7 days of perishables and 2 days nonperishable. The front and backyard are well maintained. The resident bathrooms are clean, and showers have non-skid materials. The hot water temperature measured at 116.7 degrees F, There is sufficient lighting throughout the facility. LPA observed a sufficient supply of PPE in the garage. Infection control signs were observed throughout the facility. Medications reviewed for all clients and appeared to be given as prescribed. Administrator certificate Sheila Ann Fabian # 6002001735 expire 4/20/2025. Fire extinguisher observed kitchen fully charged. There are smoke detectors/ Carbon monoxide located throughout the facility, tested and operational. The following deficiency was cited according to title 22 regulations. Appeal rights given. Exit interview conducted.
