Facility Evaluation Report

Licensing Program Analyst (LPA) Nicol Wesley conducted an unannounced Required 1 year inspection at the facility and met with staff Jasmin Raguima. The purpose for todays visit was explained. The facility phone number is 562 868 3533.

The facility consist of 5 bedrooms(2 for staff), 2 bathrooms(1 for staff), 1 living room, laundry room, Kitchen w/table, dinning room, back yard with shaded area, and attached garage.

LPA Wesley conducted a complete tour of the facility, and observe the supply of food. Resident medications, and medication logs were reviewed. The smoke detectors/carbon monoxide detector are operable. LPA observed one fire extinguisher in the kitchen area. The water temperature was tested and measured 117.9 degrees F. The last fire drill was conducted on May 19 2025. LPA Wesley reviewed 3 resident and 2 staff files. The administrator certificate for Augustus A Torres #6008452735 expires 09/02/25.

A copy of the LIC 809 was given during the exit interview.
