Facility Evaluation Report

Licensing Program Analyst (LPA) Elena Mallett conducted an unannounced Case Management visit for the facility's failure to send an Special Incident Report to Licensing regarding bed bugs being present at the facility. LPA was met by staff in charge, Irene Smith and the purpose of the visit was explained.

Per interview with Staff 1 and Staff 2 bed bugs were known to be present in a client room in the facility on 04/27/2026. No Special Incident Report had been sent to Licensing as of 04/30/2026. A deficiency was cited. See 809D.

A deficiency was cited today Per Title 22 regulations. An exit interview was conducted with staff in charge, Irene Smith and a copy of this Licensing Report along with Appeal Rights was provided.
