Facility Evaluation Report
Licensing Program Analyst (LPA) Elena Mallett arrived at the facility to conduct an unannounced Annual visit . LPA met with Administrator Vida Andrade and the purpose of the visit was explained. The facility is licensed to mentally disable adults between the ages of 18 through 59, ambulatory only.
The facility is located in the city of Norwalk. The facility consists of a living room, dining area, a kitchen, twenty three (23) client bedrooms (15 are double rooms and 8 are single rooms), ten (10) bathrooms, a laundry room, a staff room, grounds that surround the facility a covered shaded area with seating.
LPA completed the visit using Compliance and Regulatory Enforcement Tools (CARE) . The following domains were completed during the visit
Infection Control: Infection Control Plan was reviewed by LPA. Facility follows title 22 regulations and is cleaning and disinfecting regularly. Staff has PPE and practices hand hygiene.
Physical Plant and Environmental Safety: Physical plant is clean and sanitary and the hallways and walkways are free from debris and obstruction. Hot water temperature from the bathroom sink faucet measured within Title 22 regulations of 105F-120F in the six client bathrooms tested. Facility has carbon monoxide detectors that were tested and operational. Three fully charged fire extinguishers were present .LPA was provided with an official Fire Inspection Report from an inspection that took place on 06/18/26 that showed no deficiencies. Sharps and toxins and poisons are kept locked in cabinets. Six client rooms were selected at random and found to contain the required light and furnishings and linens.
