Facility Evaluation Report
Licensing Program Analyst (LPA ) Elena Mallett arrived at the facility to conduct an unannounced Annual visit. LPA met with staff in charge Wilma Hintay and the purpose of the visit was explained. Staff in charge contacted Administrator Juanito Paguio by telephone and Administrator joined the visit shortly after. The facility is licensed to serve clients between the ages of 18 through 59 one of which may be non-ambulatory.
The facility is a single-story home, located in the residential area of Norwalk that consists of a living room, dining area, kitchen, four (4) client bedrooms, (1) one staff bedroom, (2) bathrooms, detached garage, a front yard and backyard with a shaded seating area.
LPA completed the visit using Compliance and Regulatory Enforcement Tools (CARE) . The following domains were completed during the visit
Infection Control: Infection Control Plan was reviewed by LPA. Facility follows title 22 regulations and is cleaning and disinfecting regularly. Staff has PPE and practices hand hygiene.
Physical Plant and Environmental Safety: Physical plant is clean and sanitary and the hallways and walkways are free from debris and obstruction. Hot water temperature measured from the bathroom fixtures in both bathrooms did not fall within Title 22 regulations (105-120F) . A deficiency was cited. See 809-D. Toilets and sinks in bathrooms were operable but the shower mat in one of the bathrooms was observed to need cleaning. A technical advisory was given. Facility has combination smoke and carbon monoxide detectors that were tested and operational. A fully charged fire extinguisher was present. Sharps, poisons, cleaners and toxins were locked in cabinets and inaccessible to clients. Client bedrooms had required light and furnishings. Extra linens and blankets were observed. Backyard area was free of debris and obstruction and a shaded area with patio furniture was available for client to enjoy the out doors.
