Facility Evaluation Report
Licensing Program Analyst (LPA) Elena Mallett conducted the required annual inspection. LPA arrived unannounced and met with House Manager Rosanna Reyes. Licensee Carol Leiss was notified by phone of LPA's arrival. The purpose of the visit was explained to both parties. The facility is licensed to serve 8 ambulatory only adults ages 18-59. There was 7 ambulatory clients present on the day of the visit.
The facility is a single-story home located in a residential area. The facility consists of five (5) client bedrooms , one staff bedroom/office, a kitchen, dining room, living room, laundry room, 2 client bathrooms, front and back patio area, and detached garage .
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following :
Infection Control: The facility staff are using appropriate hand hygiene and gloves while assisting residents’ medications. Staff are still cleaning and disinfecting throughout the day. Facility has sufficient PPE supplies and has an Infection Control Plan .
