Facility Evaluation Report

Licensing Program Analyst (LPA) Nicol Wesley conducted an unannounced Required 1 year inspection at the facility and met with Armando Flores. The purpose for todays visit was explained. The facility phone number is 562-484-9153 land line 714 483 4895 cellphone.

LPA Wesley utilized the CARE tools as part of the inspection. The facility consist of 4 bedrooms, 3 bathrooms(1 located in room 1), living room, staff area, washer and dryer located in the garage, kitchen w/table, back yard with shaded area, and attached garage.

LPA Wesley conducted a complete tour of the facility and observe the supply of food. Resident medications, and medication logs were reviewed. The smoke detectors/carbon monoxide detector are operable. LPA observed one fire extinguisher located in the kitchen. Water was tested and measured at 119.1 degrees F. The last fire drill was conducted on February 11, 2025. LPA Wesley reviewed 3 resident and 3 staff files. The administrator certificate for Armando Flores #7011438735 expires 04/18/2026. There were no deficiencies cited.
