Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual inspection visit. LPA was greeted by House Manager Margarita Aquino and the purpose of the visit was explained. Administrator Vany Rasile arrived shortly after to assist with record review. Anita Matharu is the Licensee.
The facility is licensed to serve six (6) non-ambulatory clients ages 18-59. Currently one (1) client over the age 59. All clients receive services from Harbor Regional Center.
The facility is a single-family home located on a main street in the city of Norwalk. The home consists of a living room, dining area, kitchen, 4 client bedrooms, 2 bathrooms, office, detached garage, front and back yards.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Living room and dining area were observed clean and free of obstruction with sufficient seating. Living room contained a fireplace that is not operable. Games and activities were observed stored in office. Kitchen was observed clean and free of pests. LPA observed sufficient supply of nonperishable food for one week and perishable foods for a minimum of two days. Soaps, detergents, and cleaning supplies were observed to be stored away from food supplies. Sharps are secured in a locked kitchen drawer.
