In conjunction to the complaint 31-AS-20250721161857 LPA Mariana Agban conducted case management- Deficiencies visit. During the physical plan tour, LPA observed R1's room to be dirty. LPA observed stained carpet, unclean bedsheets on the floor, the toilet was unclean as well and a lot of clutter in the kitchen area. Exit interview conducted, citation issued and a copy of this report signed and delivered.
