Licensing Program Analysts LPA Leizl de la Cerra conducted an unannounced visit on 9/25/24 for the above noted allegation. LPA was greeted with staff caregiver, Roanne and explained the reason for the visit. Facility Administrator, Rowena Cruz was contacted and arrived shortly.

Regarding the allegation above, Community Care Licensing (CCL) received a complaint alleging that on 09/05/24 multiple medications were discovered to be unlocked and accessible to clients. Prescription medication was observed in the bathroom and hallway closet.

During investigation, at approximately 10:45 a.m. LPA de la Cerra conducted tour of physical plant and did not observe any medication to be unlocked and accessible to the residents. At approximately 11:30am LPA interviewed the facility administrator and other staff, regarding the medications being accessible to the clients. Administrators admitted that while assisting clients with bathing, staff #1 (S1) took clients individual medication baskets/containers out of the locked cabinet. S1 placed medication baskets in the hallway and in the bathroom to be able to dispence medication after bathing. During inspection, LPA observed that the hallway and the bathroom are the areas that are easily accessible to facility clients.

Based on inspection, observation and the information revealed from the staff and credible witness there is a sufficient information to support the allegation. Therefore the allegation is SUBSTANTIATED at this time. Under California Code of Regulations,Title 22, Division 6 citation was issued and recorded on the attached LIC9099-D.

LPA did not observe any health and safety issues during today's visit. Exit interview conducted. Copy of signed report issued.
