Facility Evaluation Report

Licensing Program Analyst (LPA) Mariana Agban conducted a case management visit to confirm the closure of this facility. On 6/30/26 LPA Mariana Agban received an email from the licensee regarding the facility closure.

During today's visit, LPA met with the Licensee Emma Parcia. LPA conducted a physical plant tour and confirmed that there were no residents at the facility. LPA obtained the License. The licensee initiated the closure of the facility.

LPA will submit the file for closure upon returning to the Woodland Hills South Regional Office. The effective closure date is 08/05/26.

An exit interview was conducted, and a copy of this report was signed and provided to the licensee.
