This is an official report of an unannounced visit/investigation of a complaint received in our office on

04/22/2025 and conducted by Evaluator Mariana Agban

COMPLAINT CONTROL NUMBER: 31-AS-20250422163601

Licensing Program Analysts (LPAs) Mariana Agban and Nadia Shahbizian conducted an unannounced visit to this facility. LPAs met with the Licensee Lani Manzano and explained the reason for the visit. LPAs conducted a physical plant walk through to ensure that the facility is in compliance with rules and regulations under California Code of Regulations, Title 22, Division 6. LPAs did not observe any immediate health and safety issues during the visit. Based on the information LPAs gathered LPAs determined that the allegations are unfounded. Resident#1 (R1) doesn’t live at the address stated on the complaint report. A finding of unfounded means that the allegation is either false, could not have happened, and/or is without a reasonable basis. We have therefore dismissed the complaint.
