Facility Evaluation Report

Licensing Program Analyst (LPA) Mariana Agban conducted an unannounced case management visit to conclude the investigation findings related to the self-reported incident dated September 26, 2025. LPA previously conducted a case management–incident visit on October 2, 2025, during which involved staff were interviewed and relevant records were obtained. Based on the interviews conducted and the records reviewed, there was insufficient information to verify the suspicion of physical abuse. Therefore, the allegation of physical abuse is determined to be unsubstantiated at this time.
