Facility Evaluation Report
Licensing Program Analyst (LPA) Mariana Agban conducted an unannounced CASE MANAGEMENT-INCIDENT visit to this facility and met with Administrator Lora Pana. LPA conducted a physical plan tour to ensure the health and safety of the Clients are protected and are in compliance with Title 22 Regulations.
On September 26, 2025, Community Care Licensing (CCL) received an Unusual Incident Report indicating that Staff#1(S1) heard a noise from Client#1's (C1) room. C1 had just taken a shower and went back to the room. With the supervision of (2) DSP Staff#2 (S2) and Staff#3 (S3), C1 was drying himself/herself. S2 was inside C1’s room, and S3 was directly across in C2's room. S1 reported witnessing S2 hit C1 on the neck. S1 was in C3’s room as a 2:1 Staff which was across the hall. During today's visit, LPA interviewed the Administrator, S1, S2, S5, and Case Manager from North LA County Regional Center. LPA attempted to interview S3 and S4. LPA obtained copies of pertinent information, which includes, and is not limited to, LIC 500, the Client's Roster, Staff training, and Staff Schedule. LPA is unable to close out the investigation at this time. Additional information is needed before LPA can conclude this investigation.
