Facility Evaluation Report

Licensing Program Analyst (LPA) Tihesha Smith conducted an unannounced annual visit to this facility. LPA Smith contacted the Licensee who has authorized daughter to sign, Licensee also revealed may begin operation in approximately 8 months to a year. The licensee also revealed will contact LPA or Licensing department if decision to remain open changes.

LPA Smith was greeted by Licensee daughter Ani Manvelyan and she revealed facility is still not operating.

LPA Smith conducted a tour of the physical plant to ensure there are no health and safety hazards and no clients are currently residing in the home. The only individuals in home are licensee's family.

LPA advised when decision is made to begin operation to ensure stacked bricks/rocks are removed from side patio area.
