Facility Evaluation Report
At approximately 1:00 p.m. on 03/26/25, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced annual inspection visit. LPA met with a tenant and disclosed the reason for the visit.
A file review was conducted prior to the visit.
LPA called the licensee at approximately 1:15 p.m. today who confirmed that the facility still has no clients and has yet to admit their first resident.
LPA toured the facility at 1:30 p.m. and observed no residents requiring care and supervision. Observed were two (02) adult tenants and two (02) child tenants who are all family of the licensee.
A deficiency was issued for previously unpaid annual licensing fees. Deficiency cited on the corresponding LIC 809-D page.
No immediate health or safety concerns were observed during today’s visit.
