Facility Evaluation Report

Licensing Program Analyst (LPA) Mariana Agban conducted a case management visit to confirm the closure of this facility. LPA contacted the licensee and later met with Kathleen Hopkins. On 03/04/26, LPA received an email from the licensee regarding the facility closure. During today's visit, LPA conducted a physical plant tour and confirmed that there were no clients at the facility. LPA obtained copies of the Client's Identification and Emergency Information (LIC 601) and the Individual Program Plans (IPP). On 03/12/26, LPM Flores contacted the North LA County Regional Community Services Supervisor and confirmed that all three (3) clients have been safely relocated. The licensee confirmed that clients and their responsible parties were notified of the closure. The licensee initiated the closure of the facility. The licensee was instructed to mail the license to the Regional Office.

LPA will submit the file for closure upon returning to the Woodland Hills South Regional Office.

An exit interview was conducted, and a copy of this report was signed and delivered.
