At 2:11 PM Licensing Program Analyst (LPA) Leslie Ngo-Castaneda met with Marilen Basa to conduct a Case Management Visit in response to the Licensee's request to close the facility. LPA toured all areas of the facility and observed that there are no clients requiring care and supervision. The Licensee reported that no clients was admitted to the facility. A copy of the facility license was surrendered and the facility closure survey will be mailed to the licensee.
An exit interview conducted and copy of this report provided to the Licensee.
