The inspector’s account

Licensing Program Analyst (LPA) Mariana Agban conducted an Annual Required visit and inspection of the facility. LPA met with staff Armine Manukyan, and explained the reason for the visit. Administrator, Elen Kirakosyan could not attend the annual for the day due to an emergency. Licensee Ovsanna Khayalyan came in her place. At approximately 1:15 pm, with the assistance of staff, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are interconnected and battery-operated. There is a carbon monoxide detector that functions properly installed in the hallway between the resident rooms. The fire extinguisher is located in the kitchen. The purchase date is June 06, 2025.

Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility, properly stored. Knives were stored in a locked drawer in the kitchen. Properly labeled medications were locked in one of the kitchen cabinets.

Bedrooms: There were three (3) bedrooms designated for residents' use. All three (3) bedrooms are designated for share use. Bedrooms, in use by residents, were properly furnished with appropriate beddings and linens, with sufficient lighting. BATHROOMS: The facility has one (1) main bathroom that also has an access door to Room#3 and one (1) in bedroom #1 for the residents in that room to utilize. The hot water delivered in the bathroom sink was measured at 109.4 and 111.8°F.

Common Areas: These included the living room and dining area. The common areas were properly furnished. The auditory alarms on all exit doors were on and functional at the time of the visit. Seating such as couches where in good repair and sat the capacity of the facility. There is a non-operational fireplace with a screen in the living room. No fireplace tools or fixtures present. Surrounding Grounds: Entry/exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was free of hazards.
